Really interesting! SARS-Cov-2 can induce neuronal fusion over large distances, with bridges large enough to swap mitochondria. Resulting syncytia are viable but either synchronize activity or show no activity. https://www.science.org/doi/10.1126/sciadv.adg2248 #covid #newpaper

If a medication can reduce the incidence of Long Covid, this is further evidence that Long Covid is a real medical condition:

"If a new disease has been sufficiently well characterised by clinicians so that it can be successfully modified by a therapy compared with placebo, then the entity must, from a practical standpoint, truly exist. Put differently, a treatment can only be effective if there is something to treat."

Pretty exciting: now published in The Lancet, #Covid patients who took metformin had a 41% lower incidence of #LongCovid than controls—and if treatment was initiated within three days of symptoms, the reduction was 63%!

Caveats: study population was all over 30 with BMI over 25, so may not hold for other groups.

Still pretty major news, especially as metformin is a very affordable med. Right now in the US, without insurance the script is $4 at Walmart.

Starting to think neither my wife nor I got #Covid from last weekend being around more people than anytime since the #pandemic began. More than 3/4 of infected people would have gotten symptoms by this point. (See my original post to which I’m replying for details.)

The prior probability of having been infected from our modest controlled exposures was quite low. But the stakes are very high, so I’m still relieved.

Maybe I’ll do a 🧵 here soon on cumulative risk. It’s important and forgotten.

Considering the state is doing fuck all to mitigate the spreading of covid it doesn’t matter to me if he stays or goes.

Pre-omicron study over 2 years with ~2.2k patients tracked #recovery from #COVID and persistent symptoms. 91% got better slowly (still relapses 2 years later), while 5% (mostly younger) got better faster (back to normal in 2 years) and for 4%, symptoms didn’t improve. People who got better faster more likely to report neck/back pain, diarrhea in first year. People w/ persistent symptoms had more cardiac-related symptoms, sweats, heat/cold intolerance, sensitivity to light/sound.

People using #SSRIs were 1.3x more likely to die if they got #COVID compared to non-SSRI users. (Apparently people on social media were advising to take SSRIs just to prevent COVID??) The study was with Danish adults, up to 60 days after positive RT-PCR test.

(IANAD, but I’m sure if you’re already on SSRIs it’s best to keep taking them. The lesson here is don’t take SSRIs expressly to prevent COVID…)

#TWiVTLDR 4/

…Not necessarily because the rapid antigen #tests are worse with #omicron, but because so many already have some immunity to #COVID (vaccination or infection) and our immune systems are trying to fight it off, lowering the viral load in the first few days.

#TWiVTLDR 3/

Anita Y Cheng

A rapid antigen #test (European Sofia brand) was studied in clinical setting. It was less sensitive for omicron and for ppl with lower viral loads compared to RT-PCR. Interestingly, more sensitive in men and older patients. Big takeaway is that we can’t really use RATs to screen for early asymptomatic #COVID cases anymore. Best time to use a RAT is day 2-3 of #symptoms

Still trying to catch up with #TWiV. 😅 May 18, 2023: https://www.microbe.tv/twiv/twiv-1008/

The CDC finally issued #ventilation guidance for buildings! 5 air changes per hour. To compare, negative pressure isolation rooms in hospitals have at least 12 air changes per hour. A standard hospital room has 6 air changes per hour. Dr. Griffin would love to see air changes per hour posted on public buildings to make educated decisions about #COVID risk. 1/

Reminder: The tools that protect us from #COVID — masks, air filters, and structural policies like expanded healthcare access — are the tools that we need to protect us from the #climate crisis, which is causing a decrease in air quality and increase of disease outbreaks.

Whoever needs to hear this:

.
It's ok to wear a mask.

We were the only people wearing masks anywhere, with the exception of another person wearing a KN95 at the grocery store. It was alienating and surreal. I now feel almost more like a stranger in a strange land (nod to Heinlein for his classic sci-if book title) than I did before.

WTF is wrong with everybody? Sure, there is less virus circulating in this part of the country right now. And sure, you probably won’t wind up in a hospital or morgue from #Covid if you’re vaccinated. But…

My wife and I just returned from a trip to Western Washington State, doing our best to stay free of #Covid. Mostly outdoor stuff, darting into a restaurant for takeout, using way more public toilets than we’d have liked. This after basically living like hermits for much of the past three years.

I wish I could say it was great to be around people again, but it wasn’t. I’m frustrated, tired, and so very disappointed with almost everyone.

This 🧵 is a bit of a rant for fellow #ZeroCovid people.

I flew back from England with symptomatic COVID-19. I didn't know it. I had taken the required (at the time) test at Boots the Chemist and got a false negative. I wore a 3M N95 mask, so I have hopes I didn't spread it to anyone.

Study: Stopping universal admission testing was linked to significant increases in hospital-onset SARS-CoV-2 infections. Hospital infections remain common with high mortality rates.

Hospitals should not stop universal admission testing.

May 2023:

Oh, and last:

There’s a major symptom of brain injury called anosognosia.

It’s a deficit of self-awareness and insight into one’s own condition — the brain cannot perceive the damage done to it, so the patient is unaware that they have sustained an injury.

It's not denial. It's not a sensory-specific or hemispheric issue. It's not an amnesia, or an inability to form a new memory (though it may be related). It does make regular family relationships difficult — it's often the underlying problem when it's time to take the car keys.

Word of the day: anosognosia. Rings true!

It is unfailingly depressing that the majority of #COVID illnesses and deaths are so ridiculously easily avoidable. Vaccines aside, simply wearing a mask and avoiding gatherings could essentially wipe out the virus. But the "I need to see faces" and "I miss the office" folks won't have it.

Folks will be like "I could easily walk faster than these shuffling zombies but I miss the touch of people" and intentionally walk straight into their clutching grasp. Humanity is doomed.

"More than 70% of transmissions in households with adults and children were from a pediatric index case, but this percentage fluctuated weekly," the study authors wrote. "Once US schools reopened in fall 2020, children contributed more to inferred within-household transmission when they were in school, and less during summer and winter breaks, a pattern consistent for 2 consecutive school years."

So... #Twitter is replatforming all the #Nazis, #insurrectionists, and anti-vaxxers, #Trump is allowed to run for #POTUS, ringleaders of the sedition are leaders in #Congress, nobody takes #COVID seriously anymore, and we're just gonna let the #shootings continue and the planet broil, because #America has no memory and our only concern is whether rich people can retain more yacht money. Am I caught up? Did I miss anything?

Oh yippee Doc things I may have heart complications from #COVID after blacking out and collapsing yesterday, plus a fast heart rate and being less active and short of breath.

Not only was it my first time, but manage to really fk me up. Have a feeling these heart specialist are going to be buying a new boat after this.

In docs words - you’re too young to have 50+ yo symptoms 😱
