For once, an #AI issue where I *can* speak with a certain amount of authority.

I've been hearing "expert systems outperform human #diagnostics, so pretty soon human #physicians will be obsolete" for a few years now. It's closely akin to "#airliners fly themselves these days, so what do we need #pilots for?" In both cases, people are paying attention to the best-case scenario with no understanding of the *enormous* number of how many and various the worse cases really are.

This is complicated by the fact that in most of #medicine (although not necessarily #emergency medicine, the author's specialty) and in nearly all of air travel, the best case is also the normal case. Most of the time, whatever is wrong with you can be diagnosed and treated. Almost all the time, when you get on a plane, you'll walk off at the other end of the trip as healthy as when you boarded. It's reasonable to expect those outcomes.

Not-best and not-normal cases add up really fast.

Without any false modesty whatsoever: as a #medic, I learned a truly impressive degree of clinical judgement. From the first moment I saw a patient, I had a pretty good idea of #diagnosis, #treatment, and #prognosis. (Sadly, if my initial call was "this one's not going to make it," I was almost always right. The exceptions kept me going.) I learned from the best—one of my mentors had learned *his* trade in rural Guatemala, where resources were terribly sparse and human judgement was the only line between life and death. He held back death for decades, and it came for him far too early. Gene Gibbs, RIP.

I can't code that. Neither can anyone else, and if they tell you they can, they're lying.

As a #researcher, I've done a fair amount of work in #clinical #decision #support (#CDS). The idea is simple, and valid: no human, or team of humans, can remember everything they need to know. There's simply too much knowledge for the brain to hold and recall on demand. Subtle relationships exist between disparate types of data that *nobody* knows, until we tease out the numbers. We're doing this, right now. It is saving lives and relieving suffering, right now.

The key word there is "support." Humans still absolutely, positively, 100% need to be in the loop.

Maybe that will change, someday. I'm not saying it's impossible, for two reasons. First, any time anyone says "computers will never be able to ___" they're usually proven wrong. Second, I don't want to limit my and my colleagues' imaginations. We need to stay focused, but it is a *good thing* for our reach to slightly exceed our grasp. That's how #science happens!

Just not this day, and not for many days to come. Right now, we need to keep muddling along. There's not much more human than that.

When anyone walks in the door, our first question isn't "How can we fix you?", it's "How're you gonna pay for this?"

That's barbaric.

The alt med/supplement industry pushes NDT ("natural" dessicated thyroid, so dried thyroid from pigs or cows) like crazy via naturopaths and online. It's a dangerous thing and it misleads a lot of people with thyroid disorders, particularly those that are autoimmune in origin. It's important to follow the money but also to understand why they're successful.

1-A lot of people think "natural" is better but the animal proteins in NDT can actually increase auto-antibodies and worsen immune diseases (very particularly with Graves Disease but also with Hashimoto's).

2-NDT has a very unnaturally high ratio of T3 in it for humans (pigs naturally have different rations of T3/T4 than humans). The body used T3 immediately, all of it, there's no storage mechanism (which is why Graves Disease causes hyperthyroidism, too much T3). This means you're rapidly becoming hyperthyroid and crashing on a daily basis. However, being hyperthryoid can be like being manic, it can feel good (until it doesn't). I can also lead to mania and being extremely emotionally volatile.

3-Unfortunately thyroid disorders often get badly managed by doctors who base it all on TSH levels (a really bad proxy for other thyroid levels, if a good first line of testing for thyroid disorders, though not perfect for that either since one can be euthyroid and have active Trab that indicate Graves Disease). This leaves patients under-medicated so some genuinely are finding relief via naturopaths, if in a form that's also potentially harmful (but being undermedicated is also potentially very harmful, leading to the development of other metabolic diseases). Also, because thyroid disorders are far more common in women, your everyday medical sexism can be a big factor too.

4-Some people don't metabolized T4 into T3 very well (most of this happens in the liver, including for people with normal healthy thyroids), though it's less common than support groups would lead people to think (as a way to push NDT). These people need to be taking T3 alone, NDT is not the solution for that problem.

5-It's kind of crazy we don't have a proper slow release version of T3 at this point in time considering how widespread thyroid disease is. While many people do just fine on T4 alone (by metabolizing it into T3 in the liver), there can be things that prevent this (including liver disease and conversion problems). I'd suggest a lot of this has to do with a very real disregard for the quality of life of women in many areas of society, medicine and business.

I rarely find a Mastodon medical article summary that’s useful. It’s not that people aren’t trying. It’s that it takes real expertise to read these things. The authors have agendas and the abstracts often mislead.

For example, the abstract might say X is better than Y. They won’t say the difference is clinically irrelevant.

Even in my dotage I’m good at parsing these, but I read people who are a lot better. It’s hard. #medicine

Excited to watch how hundreds of millions of dollars of pharmaceutical company advertising completely shifts public opinion on obesity being "a choice" just so they can market new GLP-1 based weight loss drugs directly to patients. In 1972, the Nobel Foundation stated his omission was a mistake.

In the ER, we routinely ask our chest pain patients if they have cardiovascular risk factors like hypertension, diabetes, ⬆️chol, etc. We have enough evidence that #COVID19 ⬆️ one’s risk of #heartattack , #arrhythmias, #myocarditis, etc. & that reinfection may confer addtl risk.
