This AI will follow along and take the session notes for the doctor by
listening to the office visit.  It will also bring up charts and records
through voice command and prompt the doctor to do routine things during
the office visit.  It's due out early next year.

This could be very helpful.

However I can imagine a few kinks in the office visit process initially:

Patient: "Doctor, my knee hurts"

AI: "REMEMBER TO MAKE A FOLLOW-UP APPOINTMENT"

Patient: "What was that?!"

Doctor: "Oh pay no attention -- that is just the new AI system everyone
has to consent to for treatment.  It will help us during the session."

AI: "HAVE YOU EXAMINED THE KNEE X-RAY YET?"

Doctor: "AI, pull up the knee x-ray"

Patient: "This is my first visit, there is no knee x-ray yet."

AI: "REMEMBER TO SCHEDULE A KNEE X-RAY"

Doctor & Patient Together: "We don't know if we need a knee x-ray yet!"

Patient: "It started hurting yesterday"

Doctor: "Jump up on the table and I'll take a look at it"

AI: "SHALL I SUMMON A NURSE TO WATCH TO GUARD AGAINST ALLEGATIONS OF
IMPROPRIETY?"

Doctor: "NO!"

Doctor: "It does look a bit red.  Does this hurt?"

Patient: "A bit when you touch there and I bend it."

AI: "SHALL I SCHEDULE THE KNEE X-RAY NOW?"

Doctor: "SHUT UP!  AI -- Silent mode now!"

Office visits are going to be fun the next few years while this gets sorted.

If you ever wonder why psychotherapists sometimes stop taking insurance
in the United States, there are lots of reasons.  Low pay is the typical
one.

Hassle is another.

I'm trying to catch-up on billing this fine Sunday afternoon. The last
thing I need is a billing rabbit hole to get stuck in...

Today's rabbit hole -- trying to figure out why one of my insurance
panels keeps paying significantly different rates on the same CPT 90837
billing code (the code for a roughly one hour session).  A LARGE
difference in fact.

Took a long time...  drum roll...

I'm paid LESS for POS 02 (place of service telehealth AWAY from home)
versus POS 10 (place of service telehealth AT home).

Cause, yeah -- that makes sense.

So, yes, I'm required to code whether or not a client is at home for
telehealth.  Why?  No idea.  (I suppose *I* do need to know the address
in case I need to send an ambulance or something.)

This all was made especially fun by:

a) The insurance company does not publish a rate schedule.  No way to
know if I'd get a check or a claw-back if I inquire about the discrepancy.

b) The client has co-insurance.  So the co-insurance rate varies by
place of service (POS) too.  Client pays a percentage (%) of the total
amount I'm due.

It's crazy-making.  And "crazy" is a term I usually avoid.  Except when
discussing the American health system.

From the abstract, looks like interpersonal psychotherapy and intensive
short-term dynamic psychotherapy are the most promising candidates.

In order to do this, I had to turn off "Brave Shields" -- basically a
web browser feature that blocks 3rd party tracking (cookies, web

turn off my 3rd party tracking blockers.**

Also -- although I will never use your Good Therapy Verified Seal widget
-- its abilities to collect data for tracking, analysis, and advertising
from mental health websites is in very poor judgement.  This stops only
just slightly short of a HIPAA violation as anyone looking at a
therapist's website is certainly considering mental health help.  Data
from multiple such widgets and trackers across websites is used all the
time by 3rd party aggregators to discover the full name and identity of
visitors.

This is disappointing behavior that has lowered my trust in your
organization.

I wrote an article about Barbie after an interview with psychiatrist/cultural theorist Dr. Jeanne Randolph for the #MedicalPost/#CanadianHealthcare Network. It's free for #doctors, #pharmacists in #Canada. But here are few salient paragraphs.

"Dr. Randolph is both a #psychiatrist and a cultural theorist who uses a psychoanalytic lens when looking at icons such as Barbie. Studying #Barbie provokes questions about toys' contribution to the #health and ethics in #kids.

Indeed, back in the 1990s, she wrote a chapter for a book, Consuming Fashions: Adorning the Trans-National Body, that included details of a small study she conducted investigating how girls treated their Barbie dolls. The book itself included historical psychological and anthropological aspects of fashion.

What she found in wasn’t pretty.

Indeed, it was common for girls to cut off Barbie’s hair, paint them, poke or burn holes in them, twist their head around backwards, remove limbs, or even swap their head with other dolls.

But, she told the Medical Post, it's healthy for young kids to express aggressive or sadistic behaviours through their toys. It helps them cope with, and understand, their aggression in the long run—better than keeping it all bottled up and having it expressed in something real.

One moment, Barbie is a super model, an ideal parent or a brilliant astronaut, the next she might be beating up other toys, joining a biker gang, or holding up banks. Young kids learn about themselves and the world around them through play, she said.

For a long time, Barbie was marketed with an image of femininity that was devoid of aggression.

Barbie represented a huge cultural shift when she was first introduced in 1959. Prior to that, girls were given dolls that looked like babies, making their play limited to that as moms or caregivers—a fact the new movie points out.

Barbie was the first adult doll girls could identify with, Dr. Randolph said. They could expand the roles they played—except she was presented an exaggerated version of some idealized female form with her tall, slender proportions.

“In a way, Barbie was asking for it. It’s like the Barbie doll is so perfect, so unrealistic that she incited children to resent her or to be anxious because she’s not real,” she said. Hence some of the destructive behaviour.

Yet doing things like removing a Barbie’s head doesn’t mean a child will grow up a sociopath. Often, it’s just because the heads are easy to remove, and more of an exploration of how things work.

When she wrote about Barbie for that book chapter in the 1990s, Dr. Randolph wanted to include a picture of the doll. She looked for an advertising image that showed some aggression in Barbie, but the best she could find was an image of a fishing Barbie holding a large, plastic dead fish. The fish had a smile on it.

She wrote the toy company that produced Barbie for permission to use the image.

“I got a letter back from Mattel’s lawyers. They had read my chapter all about sadism and about how Barbie is asking for it, the doll is too perfect. They said I should seek legal council. They said they entirely rejected the negativity of this chapter and that Barbie is a valuable product which they did not want to be seen in this light,” she said.

There was always a disconnect between how Barbie was marketed and how little girls played with her.

The chapter was published, and the letter from Mattel was included in its entirety in a footnote.

However, the new Barbie movie showed a turnabout in recognition of the dichotomy Barbie represents. It acknowledged the imaginary, perfect world Barbie comes from, the cultural impact of Barbie dolls, that in real-life Barbies are mistreated, that many kids have cynical attitudes about Barbie, and also that some people love her.

“The movie is exactly what you would hope for in terms of the image and the humanity of women,” she said. The #feminist messages are strong, with powerful satirical punch towards #patriarchy."

My surprise face when a year after joining, theres a noted dearth of #Psychiatry content on the #Fediverse.

I was invited by a local county judge to present my caregiver perspective at a regular monthly meeting of the Behavioral Health and Criminal Justice Advisory Committee of Travis County, Texas. The group's goal is reducing the entanglement of persons with mental illness in the criminal justice system. My wife joined me in presenting, yesterday, and we both thought it went great! We spoke about our personal experiences as parents and caregivers for an adult living with schizophrenia and shared data from author Seth Stoughton, the Police Executive Research Forum (PERF), and the CAHOOTS program in support of improving the response to those in crisis. The committee members were engaged and we received enthusiastic applause at the end of our presentation. I hope we made a lasting difference. Our five wrap-up suggestions:

1. Reduce the contact persons with serious mental illness have with the justice system:
a. Certified Peer Support Specialists could be hired to conduct frequent telehealth wellness checks and paid a stipend by the minute for the time they spend on their cell phones with clients.
b. Caregivers could benefit from de-escalation training and access to help from outside the justice system, perhaps from a network of family and friends who undergo training to become Certified Family Peer Specialists.

2. Pledge that first-responders will acquire skills for an unarmed response to those without a gun. A mutual “no guns” agreement would require that no gun is available to a person with serious mental illness for there to be a private response. This would increase the safety of all stakeholders. Such a program is one that caregivers could get excited about.

3. Minimize coercion by making hospitality a leading feature of the diversion center. Make it a “living room” where persons want to go for help. Make it a place all stakeholders will encourage persons with mental illnesses to go for help. We don’t want any stakeholders to discourage use of the diversion center.

4. For those who hesitate to accept treatment, offer financial incentives for psychotic and homeless persons to try a long-acting injectable antipsychotic, like the incentives offered to blood plasma donors and in clinical trials of new medications—if they are competent to reject treatment, they are competent to accept treatment with an incentive.

5. Don’t forget the <1% with unstable psychotic disorders who have lost reality contact and are homeless. Unless they are already known to be non-responders to prior treatment, consider a brief involuntary commitment at a facility separate from the diversion center and with the approval of collaborating judges and physicians. Develop new ways to improve the lives of treatment non-responders.

Marcus Hutchins at Malwaretech posted about a new "feature" of Chrome that reports to 3rd parties what websites you visit.

This means YOUR CLIENTS can have the URLs of your TELEHEALTH system reported to 3rd parties.

So -- for example, if you use Psychology Today for telehealth, they will know the client is seeing a mental health professional. If you use Zoom, they will LIKELY just know the client went on a Zoom call -- but then you have a unique Zoom URL link, so its possible someone will bother to catalogue that your particular Zoom link is medical. So, again, same problem.

This is not a HIPAA problem under your control, but perhaps some client education on browser privacy settings is in order?

Psychiatry in the west has been built upon a long history of colonial aggression. If you are part of a minority group, chances are you will end up in a psychiatrists office at some point in your life.

Psychiatry then positions the problem in your brain, rather than the political and material circumstances of your life.

One of Iowa’s largest cities repealed its ban on “conversion therapy”… after a Christian organization threatened legal action, part of a deepening national movement to challenge protections for LGBTQ+ kids.

The city council in Waterloo voted this week to remove its restrictions after Liberty Counsel warned in a letter June 30 that it would “take further action” if the city did not repeal the ordinance by August 1. It was enacted in May.

The organization, which is based in Orlando, Florida, argued the ordinance infringes on the constitutional right to free speech and acted on behalf of a therapist in Waterloo “who was concerned about the implications of this on the practice of counseling,” Mat Staver, Liberty Counsel founder and chair, said in an interview in which he promised further litigation targeting states.

In Iowa and across the country, efforts are spreading to curb the rights of LGBTQ+ kids and adopt restrictions on gender and sexuality in classrooms, youth sports and medicine.

In recent years, local bans on conversion therapy in Florida also fell with the help of Liberty Counsel, which describes itself as a Christian ministry that is “restoring the culture by advancing religious freedom, the sanctity of human life and the family.”

Such therapy has been discredited and is opposed by, among others, the American Medical Association and the American Psychiatric Association, citing research that shows it leads to increased risk of suicide and depression.

“The mental health mainstream believes that one, that these practices don’t really work, and two, that they may cause harm,” said Dr. Jack Drescher, a Columbia University psychiatry professor and editor of the chapter on gender dysphoria in the psychiatric association’s diagnostic manual.

Laws prohibiting mental health professionals from attempting to change a minor’s sexual orientation or gender identity are on the books in 22 states and Washington, D.C., according to the Movement Advancement Project, an LGBTQ+ rights think tank. In 13 states, including Iowa, some municipalities have adopted their own provisions.

The issue has the potential to come to a head in the next year if the U.S. Supreme Court decides early this fall to hear the appeal of a Washington state therapist, Brian Tingley, whose lawsuit was dismissed.

While early lawsuits similar to Tingley’s failed, a Supreme Court ruling in 2018 prompted a new round of cases, said Christy Mallory, legal director at the UCLA School of Law’s Williams Institute, which researches sexual orientation and gender identity. That ruling invoked free speech protections to block a California law that required anti-abortion centers to provide information about abortion.

In 2020, a panel of three federal judges in Florida relied in part on that 2018 ruling and became the first federal appeals court to block ordinances in Boca Raton and Palm Beach County that banned conversion therapy. Liberty Counsel represents the two therapists who won that case.

The diverging federal rulings in the Washington and Florida cases may be a reason for the U.S. Supreme Court to weigh in this term, bringing another high-profile LGBTQ+ issue to the docket.

Staver is confident the Supreme Court will strike down bans in the near future. And Liberty Counsel has imminent plans to sue over statewide bans, he said.

“I think it is a losing proposition for any state or local government to have one of these laws, and they would be wise to repeal them before they also are sued,” Staver said.

In Iowa, Senate Democrats and a Republican in the House introduced bills for conversion therapy bans that didn’t make it out of subcommittees in 2020. That was the last time there was a concerted effort for a ban in the state, said Damian Thompson, public policy director at Iowa Safe Schools, an organization that advocates for LGBTQ+ children.

Meanwhile, in Iowa and elsewhere, laws have since been passed to prohibit teachers from raising gender identity and sexual orientation issues with students through grade six, to restrict the restrooms transgender students can use, and to ban treatments like puberty blockers and hormone therapy for trans minors. Many are facing challenges in court.

Republican lawmakers say the laws are designed to affirm parents’ rights and protect children. The issues have become flashpoints in the race for the 2024 Republican presidential nomination.

But many parents and advocates worry about the deterioration of the rights and safety of LGBTQ+ kids.

In Waterloo, a city of over 65,000, Councilor Jonathan Grieder said he had heard concerns about conversion therapy happening in the area. He worked with Thompson at Iowa Safe Schools to draft an ordinance after watching statewide efforts stall but a ban pass in another Iowa city, Davenport, and in Linn County.

The Waterloo council approved it 6-1 in May, but overturned it 4-3 on Monday amid the prospect of costly litigation.

Archer Trip, of neighboring Cedar Falls, addressed the council before the repeal vote as a “survivor of conversion therapy” who had been placed there in high school.

“It does not work. Now, I am a proud queer man, but I am also here to protect everyone else,” Trip said. “We should protect our children.”

Archer’s twin sister, Nic Trip, who was also put in conversion therapy, testified: “Unfortunately, not all parents always make the safest decision for their children. What is the line of what is OK to do to our children?”

Mayor Quentin Hart said, not mentioning the Liberty Counsel, that there was “threat of impending litigation moving forward,” which put the members in a “tough situation.”

“I don’t believe that the Waterloo City Council are cowards,” Hart said. “I believe that they do have a decision to make tonight.”

The decision disappointed Thompson, who said Iowa Safe Schools will continue to advocate for local bans despite far-right groups’ success in turning a “common sense” issue into a “wedge culture war” one.

“Which is a shame,” Thompson said, “because in the meantime it only results in more kids being victimized and more kids, ultimately, receiving lifelong trauma.”

A journal entry

Trying to reduce my #Methylphenidate (#concerta/ #ritalin) intake as I have built up quite a tollerence to it, the other day I had to take 70mg of concerta and multiple doses of 10mg ritalin throughout the day, so I think like 120mg tottal?

Today I just took my concerta I could feel it were off like 3hrs later even though it supposed to last 12, but I didn't take anymore today,

And God fucking Damm I was a cranky bitch this arvo, like I was just so irritable.

I have talked to my doctor about it and he said its okay to take breaks of the medication and actually recommend to take break off it when I'm not working.

Thinking about no meth tommorow to see how I go

Also thinking about maybe talking to my doc about switching to #dexamphetamine since I'm so used to Methylphenidate now

scientists have found NO correlation between net worth and IQ. there is only a weak correlation between income and IQ. the strongest correlation is found between school grades and IQ.

so it doesn't help your wallet very much to be smart. being smart doesn't make you care about having a career or making money. nor does every smart person have the circumstances or personality traits necessary for success.

it's a very relevant observation for me personally, because i was always told i was smart, and a clinical test i took as an adult to get an #ADHD diagnosis confirmed that i was smart, but my enthusiasm for school and work has only decreased steadily over time, and was pretty much gone by high school, so i never went to college. i also got bored with slaving away in an office as a computer programmer (self-taught), so i quit doing that in 2020.

A neurological take on how people end up believing #conspiracy theories -- and then get deeper into them.
Basically, "cognitive linguist" approaches (leaders using the right phrases/words) hack people's brains to get them to form opinions against political targets. Vulnerable people (those feeling uncertainty or fear) relate to what 's said, become less critical -- and real changes are made in the #brain. Those changes become stronger over time and can become more hardwired.
Much of it is about dehumanizing people -- using insults and negative words.
It can happen to people on any part of the political spectrum.
(I think one key is to be skeptical of speeches, articles etc. where insults and a lot of emotional words are used. Plus, watch for wobbly facts.)

data leaks from HIPAA BAA subcontractors/ - whether or not they end up
being legally PHI, and despite the fact that not too many therapists are

My interpretation of this article is that hospitals, clinics, insurance companies, etc. need to get links and repost icons for Facebook, Twitter, etc. OFF their websites. If you work for a big institution -- talk to your marketing team as they are used to doing this routinely. If you are a small provider, look at your website -- especially if you created it years ago back when no one thought of the problems and you just wanted some traffic.

I am glad to see this common-sense ruling by the California Supreme Court to reject a legal challenge mounted by Disability Rights California (DRC)* against Governor Gavin Newsom's CARE Act.

From the article:

"Supporters of CARE Court say it will revolutionize California’s approach to treating those with a diagnosed psychotic disorder and addiction, many of whom are also homeless. Once implemented, the program will allow family members, behavioral health providers and medical professionals, among others, to petition a judge to order an evaluation and, if warranted, a treatment plan that might include medication and housing."

“This ruling holds significant importance for thousands of Californians and their families experiencing untreated serious mental illness and substance use disorders,” Newsom said. “The CARE Act was passed with overwhelming, bipartisan support from the Legislature, and has been endorsed by a broad coalition of partners committed to implementing this transformative initiative in communities across California.”

*(DRC and similar organizations in other U.S. states are federally funded programs that have historically favored the right of individuals to reject treatment for serious mental illness, even when they have lost insight and are homeless due at least partly to the symptoms of untreated psychotic disorders. Although the DRC website does not seem to provide budget numbers for the sources of their funding, the Disability Rights Texas (DRT) website indicates 80% of DRT funding comes from federal tax dollars. It is frustrating to parent caregivers like myself that federal tax money is used to oppose efforts by parents and other family caregivers to obtain care and treatment for relatives with major psychiatric illnesses who are highly over-represented among those homeless, imprisoned, or dead due to neglect and violence.)

I am informed that a new product called #Mentalyc has entered the market. It's mission is to write psychotherapy notes for clinicians AND to gather a non-identifiable dataset for research into clinical best practices.

I have no firm opinion yet on Mentalyc, but it's expensive ($39-$69 per month per clinician) and I'd personally need to know a lot more about what's in that dataset and who is benefiting from it.

**So I'm asking the community for thoughts on what acceptable ethical and practical criteria would be for an AI to write psychotherapy notes or medical notes.**

Here are MY thoughts so far:

1) REQUIRED: The AI either:
1a) Invents NOTHING and takes 100% of the information in the note from the clinician, or

1b) Prompts the clinician for additional symptoms often present in the condition before writing the note, or

1c) Presents a very clear information page before writing that lets the clinician approve, delete, or modify anything the AI got creative with and was not told explicitly to include. (So, for example, in an experiment with Bard a clinician found that Bard added sleep problems as an invented symptom to a SOAP note for a person with depression and anxiety. This is a non-bizarre symptom addition that makes lots of sense, is very likely, but would have to be approved as valid for the person in question.)

2) OPTIONAL: The AI is on MY computer and NOT reporting anything back to the Internet. This will not be on everyone's list, but I've seen too many #BAA subcontractors playing loose with the definition of #HIPAA (medical privacy) and there is more money to be made in data sales than clinician subscriptions to an AI.

3) OPTIONAL: Inexpensive (There are several free AI tools emerging.)

4) OPTIONAL: Open Source

5) Inputting data to the AI to write the note is less work than just writing the note personally. (Maybe a complex tablet-based clickable form? But then, a pretty high percentage of a note can be in a clickable form format anyway.)

6) The AI does NOT record the entire session and then write a note based upon what was said. (It might accept dictation of note directions sort of like doctors dictate notes to transcribers today.)

I think I may be envisioning a checkbox and drop-down menu form along with a space for a clinician to write a few keywords and phrases, then the AI (on my laptop) takes this and writes a note -- possibly just a paragraph to go along with the already existing form in the official note. I think. It's early days in my thinking.

Are reward responses in the human brain modulated by gut hormones?

In our systematic review & meta-analysis, we find converging evidence for enhanced reward signals by ghrelin. For GLP-1, the convergence is too low and might be dependent on the phase (anticipation vs. consummation).

Thad Tatum speaks on the need to understand white supremacy, racism, misuse of psychiatry as a tool to subjugate #BlackAmericans, keep them "in line" and into the Carceral System. As a victim of it for 28.5 years, he's very clearly inferring this is a legacy of #slavery and to be mindful of this.

The predominant theory in neuro-psychiatric diseases/disorders is the "chemical-imbalance" hypothesis. For depression, the culprit is low levels of serotonin which has of course led to blockbuster sales of SSRI drugs. We are now, safe and sad to say, a drug addled society.

However, increasingly, studies are showing that serotonin levels alone can neither be the cause nor sufficient for occurrence for depression.

So, what if any are are the necessary and sufficient conditions to cause chronic depression?

The article goes through a few, none more prominent or critical than the other: different chemical imbalance (old tea in a new pot), genetic predisposition (only possibly accounting for 5% of the etiology), environmental factors like stress and other cues (especially to activate such a genetic predisposition), different and fewer neural connectivity than "normal", chronic inflammation that might lead to such fewer connections, and neuroplasticity changes/malfunction as the reason for inflammation and fewer connections.

Are all of the above factors causes? If so, are we classifying multiple different conditions into a single rubric called depression (which the article terms, "the umbrella theory" with a nod to oncology and cancer classification). Can we classify better? Are we missing other crucial factors?

Was depression, historically, as high as it is today? (I know we don't have much data, but let's overestimate how many were depressed centuries ago, was it as high as it is currently?). What about the ways we have structured our society and political-economy with rabid individualism and alienation at its core? Are they not potential causes of depression?
